Please indicate your disagreement (SD=strongly disagree, D=disagree) or agreement (SA=strongly agree, A=agree) with each item. N means neutral, i.e. you neither disagree or agree. [Put your mouse over Show Scale and the scale definitions will pop up.]
Please review review your Worklife form annually and ensure that the rating accurately reflect your current level of satisfaction. Your College will use the data to monitor the "mental health" of your profession and take appropriate interventions should issues arise.
